Abstract
การวิจยัครั้งนี้มีวตัถุประสงคเ์พื่อศึกษา 1) ระดับความสุขในการทำงานและความผูกพันต่อ
โรงพยาบาลของแพทย์สังกัดสำนักงานสาธารณสุขจังหวัดลพบุรี2) ปัจจัยส่วนบุคคลที่มี
ความสัมพันธ์กับความสุขในการทำงานและความผูกพันต่อโรงพยาบาลของแพทย์สังกัดสำนักงาน
สาธารณสุขจังหวัดลพบุรี 3) ความสัมพันธ์ระหว่างความสุขในการทำงานกับความผูกพันต่อ
โรงพยาบาลของแพทย์สังกัด สำนักงานสาธารณสุขจังหวัดลพบุรีและ4) ปัจจัยเชิงลึกที่ทำให้แพทย์
ไม่มีความสุขและความผูกพันต่อโรงพยาบาล
วิธีวิจัยใช้แบบผสมผสานทั้งเชิงปริมาณและคุณภาพ การวิจยัเชิงปริมาณใช้แบบสอบถาม
ประชากรเป็นแพทย์สังกัดสำนักงานสาธารณสุขจังหวัดลพบุรีจำนวน 124 คน ได้รับแบบสอบถาม
ที่ตอบสมบูรณ์กลับคืน จำนวน 115คน คิดเป็นร้อยละ 92.74 การวิจัยเชิงคุณภาพใช้การสัมภาษณ์
เชิงลึก แบบกึ่งโครงสร้างเป็นแพทย์จำนวน 15 คน ใช้การเลือกแบบเจาะจง การวิเคราะห์ข้อมูลเชิง
ปริมาณใช้สถิติค่าความถี่ค่าร้อยละค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และการทดสอบสมมติฐาน
โดยใช้สถิติการทดสอบค่าทีการวิเคราะห์ความแปรปรวนทางเดียว และการวิเคราะห์ค่า
สัมประสิทธิ์สหสัมพันธ์แบบเพียร์สัน
ผลการศึกษาพบว่า 1) ความสุขในการทำงานของแพทยอ์ยู่ในระดับปานกลางค่าเฉลี่ยที่
3.62และรายด้านอยู่ในระดับ สูงคือ ด้านสุขภาพดีด้านสุขภาพเงินดีด้านน้ำใจดีด้านจิตวิญญาณดี
และด้านใฝ่รู้ดีอยู่ในระดับ ปานกลางคือ ด้านสังคมดีด้านครอบครัวดีด้านการงานดีและด้านผ่อน
คลายดีตามลำดับ ความผูกพันต่อโรงพยาบาลอยู่ในระดับปานกลางค่าเฉลี่ยที่ 2.93 และรายด้านอยู่
ในระดับปานกลางคือ ด้านบรรทัดฐาน ด้านจิตใจและด้านการคงอยู่ตามลำดับ 2) ปัจจัยส่วนบุคคล
มีความสัมพันธ์กับความสุขในการทำงาน คืออายุสถานภาพสมรส ตำแหน่ง สาขา/ความเชี่ยวชาญเฉพาะด้าน ระยะเวลาปฏิบัติงาน แต่ปัจจัยส่วนบุคคลไม่มีความสัมพันธ์กับความผูกพันต่อโรงพยาบาล 3) ความสุขในการทำงานไม่มีความสัมพันธ์กบัความผูกพันต่อโรงพยาบาล 4) ปัจจัย
เชิงลึกที่ทำให้แพทยไ์ม่มีความสุขในการทา งานและความผูกพันต่อโรงพยาบาล คือภาระงานหนัก
เมื่อเทียบกับรายได้หรือค่าตอบแทน มีทางเลือกที่ดีกว่า เบื่อหน่ายระบบราชการขาดความสมดุล
ระหว่างชีวิตกับ งาน การประเมินผลงานวิชาการเพื่อเลื่อนระดับ ไม่ผ่าน ปัญหาการทำงานเป็นทีม
และการย้ายติดตามครอบครัวหรือกลับภูมิลำเนา
ดังนั้น ผู้วิจัยจึงมีข้อเสนอแนะให้ 1) คณะกรรมการบริหารโรงพยาบาล องค์กรแพทย์และ
ผู้เกี่ยวข้อง นำผลการวิเคราะห์ใช้เพื่อวางแผนงาน/โครงการ ส่งเสริมความสุขและความผูกพันด้านที่
อยู่ในระดับต่ำ ปานกลาง ให้สูงมากขึ้น ด้านที่อยู่ในระดับ สูงควรรักษาสภาพและพัฒนาให้ดียิ่งขึ้น
2) ส่งเสริมให้แพทย์มีบทบาททางการบริหารเพื่อสร้างความผูกพันต่อโรงพยาบาล 3) สร้างขวัญ
กำลังใจและเชิดชูเกียรติแพทย์ที่มีผลงานเด่นทั้งด้านการรักษาและวิชาการ
สำหรับนักวิจัยในอนาคตนั้นควรศึกษาเชิงเปรียบเทียบระหว่างแพทยใ์นแต่ละช่วงอายุ
(Generation) เพื่อให้ได้มาซึ่งข้อมูล ตัวแปรความสุข ความผูกพัน ที่มีความเหมาะสมทันยุคสมัย
และสถานการณ์ปัจจุบันของประเทศไทย
This study has 4 purposes: 1) to study level of work happiness and engagement of Doctors in the Lopburi Provincial Public Health Office; 2) to study relationship between personal characteristics, work happiness, and engagement of doctors in the Lopburi Provincial Public Health Office; 3) to study relationship between work happiness and engagement of doctors in the Lopburi Provincial Public Health Office; and 4) to study the factors that cause unhappiness and disengagement at work of doctors in depth. This study applied mixed method. First, the quantitative method was done by using questionnaire. 124 doctors in the Lopburi Provincial Public Health Office were provided a questionnaire and 115 questionnaires were returned (return rate = 92.74%). Collected data was analyzed by using statistical analysis including average, frequency, standard deviation, and hypothesis testing by using T-test, One way ANOVA and Pearson's correlation coefficient. Afterward, the qualitative method was done by using in-depth, semi-structured interview. 15 participants were selected purposively. The result indicated that 1) work happiness of doctors was average at 3.62 score. High level factors were happy body, happy money, happy heart, happy soul, and happy brain, respectively. Average level factors were happy society, happy family, happy work-life, and happy relax, respectively. Work engagement was in average at 2.93 score. The factors that were ranked in average level were normative commitment, affective commitment, and continuance commitment, respectively. 2) Personal characteristics (including age, marriage status, position/specialization of doctors, and work duration) and work happiness were related while personal characteristic and work engagement were not related. 3) Work happiness and work engagement were not related. 4) Factors that caused the unhappiness and disengagement of doctors at work were workloads compared with income or compensation, other work options, tediousness of bureaucracy, unbalanced work-life, unapproved academic performance evaluation, team working problems, and family settlement or hometown repatriation. Suggestions from this study include three aspects. First, board of director of the hospital, medical staff organization, and related person should apply the results of this study in planning and developing programs that increase work happiness and work engagement in factors that ranked in low and average level. Additionally, factors that ranked in high level should also be considered for further development. Second, doctors who are in administrative positions should be promoted in order to develop engagement with the hospital. Finally, morale, encouragement, and rewards should be increased for doctors who have high performance in medical treatment and academic accomplishment. For further research, the comparison between doctors in different generations can be investigated in order to attain more data, happiness factors, and engagement factors that will up to date and serve the needs in current situation in Thailand.
