Abstract
งานวิจัยชิ้นนี้มีวัตถุประสงค์ 1) เพื่อศึกษาสถานการณ์ปัญหาการทารุณกรรมทางเพศที่เกิดกับเด็ก 2) เพื่อศึกษาบทบาทของครอบครัวในการดูแลเด็กก่อนมีการทารุณกรรมทางเพศ 3) เพื่อศึกษาการจัดการของเด็กระหว่างมีการทารุณกรรมทางเพศ และ 4) เพื่อศึกษาบทบาทของครอบครัวในการจัดการปัญหาเมื่อรับรู้ว่าเด็กถูกทารุณกรรมทางเพศ
ใช้วิธีการศึกษาเชิงคุณภาพ ศึกษาเอกสารและสัมภาษณ์เชิงลึก มีผู้ให้ข้อมูลสำคัญ 2 กลุ่ม
คือ เด็กอายุระหว่าง 11-15 ปี ที่ถูกทารุณกรรมทางเพศ และเข้ารับบริการ ณ โรงพยาบาลภูมิพลอดุลยเดช กรมแพทย์ทหารอากาศ จำนวน 12 ราย และครอบครัวหรือผู้ดูแล จำนวน 12 ราย รวมเป็น 24 ราย มีการวิเคราะห์ข้อมูลโดยจัดหมวดหมู่ข้อมูล และตีความ ควบคู่กับการเทียบเคียงทฤษฎีที่เกี่ยวข้อง และบริบท
ผลของการศึกษาพบว่า 1) ปัญหาทารุณกรรมทางเพศนับเป็นสถานการณ์ที่มีความรุนแรง เนื่องจากมีการล่วงล้ำไปในอวัยวะเพศของเด็ก สามารถแบ่งเด็กได้เป็น 2 กรณี คือ กรณีที่เด็กยินยอมมีเพศสัมพันธ์ และ กรณีที่เด็กไม่ยินยอมมีเพศสัมพันธ์ ส่วนใหญ่เป็นการยินยอม โดยผู้กระทำมักอยู่ในฐานะแฟน ปัจจัยที่ทำให้เกิดเหตุได้ง่าย คือ ดูแลของครอบครัว เทคโนโลยีการสื่อสารที่เอื้ออำนวยให้เด็กกับผู้กระทำติดต่อกันได้ง่ายและรวดเร็ว เด็กไม่มีความรู้เกี่ยวกับเพศสัมพันธ์ที่เหมาะสม ไม่รู้จักการป้องกันระมัดระวังตนเอง มีทักษะเกี่ยวกับการจัดการสถานการณ์และการปฏิเสธต่ำ เด็กยินยอมด้วยความรักและไว้ใจผู้กระทำจากความสัมพันธ์ที่ใกล้ชิด 2) สถาบันครอบครัวในปัจจุบันค่อนข้างอ่อนแออันเนื่องมาจากปัญหาทางเศรษฐกิจที่ผลักดันให้ครอบครัวต้องดิ้นรนเพื่อความอยู่รอด โครงสร้างของครอบครัวเปลี่ยนแปลงไป กลายเป็นครอบครัวแตกแยกมากขึ้น ไม่เข้าใจพัฒนาการและขาดทักษะการดูแลเด็กที่เหมาะสม ไม่สอนความรู้จำเป็นเรื่องเพศสัมพันธ์ให้แก่เด็ก โดยรูปแบบการดูแลของครอบครัวที่เสี่ยงให้เกิดการทารุณกรรมทางเพศเด็กมี 2 ประเภท คือ การเลี้ยงดูแบบปล่อยปละละเลยและการเลี้ยงดูแบบเข้มงวด 3) เมื่อเกิดการทารุณกรรมทางเพศเด็กไม่มีอำนาจในการต่อรองเรื่องคุมกำเนิดแม้เด็กกับผู้กระทำจะอยู่ในฐานะแฟนที่สามารถต่อรองได้ เมื่อเกิดเหตุแล้วเด็กส่วนใหญ่เลือกที่จะปกปิดเนื่องจากกลัวครอบครัวลงโทษ ครอบครัวต้องเป็นผู้สังเกตเอง ทั้งนี้สัมพันธภาพที่ดีของเด็กและครอบครัวเป็นปัจจัยสำคัญที่ทำให้เด็กมีแนวโน้มยอมเล่าเหตุการณ์ที่เกิดขึ้น 4) เมื่อครอบครัวทราบเรื่องครอบครัวจะตกอยู่ในช่วงปรับตัวไม่ทันอยู่ระยะหนึ่ง หลังจากนั้นครอบครัวจะมีบทบาทเจรจาต่อรองกับผู้กระทำ แจ้งความเพื่อดำเนินคดีตามกฎหมาย และตัดสินใจเรื่องใช้ยาฝังคุมกำเนิด บทบาทที่สำคัญและพบว่าเป็นปัญหา คือ การสร้างข้อตกลงเพื่อปรับพฤติกรรมและดูแลสภาพจิตใจของเด็ก เนื่องจากการเลี้ยงดูของครอบครัวที่มีมาตั้งแต่ต้น มีความห่างเหินในความสัมพันธ์ดั้งเดิม ประกอบกับสภาพจิตใจที่ยังกระทบกระเทือนอยู่กับปัญหาใหม่ที่เกิดขึ้น ยิ่งทำให้มีความเสี่ยงที่สัมพันธภาพจะแย่ลง ครอบครัวมีแนวโน้มใช้การเลี้ยงดูเด็กแบบเดิมๆ ซึ่งไม่สามารถแก้ไขปัญหาระยะยาว จำเป็นต้องมีหน่วยงานให้คำแนะนำและเข้าไปมีส่วนร่วมในการเยียวยาให้ครอบครัวกลับเข้าสู่ภาวะปกติและมีแนวโน้มจะพัฒนารูปแบบครอบครัวที่ดีกว่าเดิม
ข้อเสนอแนะ 1) ควรให้ความสำคัญกับสถาบันครอบครัว โดยเสริมความรู้ ทักษะจำเป็นสำหรับเลี้ยงดูบุตรวัยรุ่นให้แก่ครอบครัว รวมถึงการจัดการปัญหาเมื่อเกิดการทารุณกรรมทางเพศเด็กแล้ว 2) ควรให้ความรู้เกี่ยวกับความเสี่ยงจากการมีเพศสัมพันธ์ วิธีป้องกันโรค การคุมกำเนิด รวมถึงการเสริมสร้างทักษะการปฏิเสธแก่เด็ก 3) ควรประชาสัมพันธ์แหล่งให้ข้อมูล แหล่งให้ความช่วยเหลือ และแหล่งให้คำปรึกษาแก่เด็กและครอบครัวให้เป็นที่รู้จักแพร่หลาย
This research aimed 1) to study the situation of sexual abuse problem with children. 2) to study the role of the family in child care before the sexual abuse. 3) to study the management of child during sexual abuse and 4) to study the role of the family members in managing problems when children were sexually abused. The research was conducted through a qualitative approach, in which documentary study and in-depth interview were used. Key-informants covered two groups: one group of 12 children aged 11-15 years who were sexually abused and went to consult with physicians at Bhumibol Adulyadej Hospital and another group of 12 family members or caregivers, totally 24 key-informants. Obtained Data of documents and interview conversations were interpretated and analyzed with the relevant context and social context. The results of the study revealed that: 1) Sexual abuse has been a severe situation due to the intrusion into the sexual organs of a child. There were cases in which a child consented to have sex or did not to, but in most cases the child consented to have sex and the actor are often a boyfriend. One Factor that makes the cause easy was the lack of care form the family, another factor was the communication technology makeing it easy for children to interact with each other. The children did not have a proper sexual knowledge, self-defense precaution and with low-level situational awareness and management skills. The child agreeed with love and trusted the person from the close relationship. 2) Current family institutions are relatively weak due to economic problems that have pushed families to struggle for survival. The structure of the family has changed to be fragmented. The family members did not understand the development and lacked proper childcare skills. Moreover, They did not teach the necessary sexual knowledge to children. By caring for a family at risk of sexual abuse, there are two types of child abuse: neglected parenting and strict parenting. 3) When a child was sexually abused, he or she had no power to negotiate contraceptives. When it occurred, most children choose to hide because of fear of family being punished. Thus, the proper relationship between the child and the family is a major factor in the child's ability to tell the story. 4) When the child's family knew about it, they would not fall into the adjustment phase for a while. After that, the family would have to negotiate with the offender and inform the lawsuit for legal action and decided to use contraceptive pills. An important and problematic aspect is the creation of an agreement to adjust the child's mental and behavioral status due to the raising of a family from the beginning. There is a distance in the original relationship. The mental state is also affected by new problems, more likely the relationship will worsen. The family raising children is more liable to use tradition which cannot fix long-term issues. There is a need to involve in the remediation of the family and to guide the development of a better family model. Recommendations for solving problems: 1) Family institution regarding knowledge and skills related to raising children should be strengthened, and problem management when child sexual abuse occurred should be reinforced; 2) Educate Children about risks of having sex, disease protection, birth control and skills to refuse sexual abuse; 3) Should publicize the source, help desk and counseling for children and families to be widely known.
