การศึกษาปัจจัยสู่ความสำเร็จการจัดการบริการสุขภาพของโรงพยาบาลแพทย์แผนไทยต้นแบบ จังหวัดสระแก้ว

Abstract

งานวิจัยนี้ศึกษาปัจจัยสู่ความสำเร็จในการจัดบริการสุขภาพของโรงพยาบาลแพทย์แผนไทยต้นแบบจังหวัดสระแก้ว มีวัตถุประสงค์ เพื่อศึกษาปัจจัยสู่ความสำเร็จ ปัญหาและอุปสรรคการจัดบริการสุขภาพแพทย์แผนไทยโรงพยาบาลวังน้ำเย็น จังหวัดสระแก้ว ใช้การวิจัยเชิงคุณภาพ โดยการสัมภาษณ์เชิงลึกผู้ให้ข้อมูลสำคัญรวมทั้งสิ้น 41 คน จำแนกออกเป็น 4 กลุ่ม ได้แก่ กลุ่มผู้กำหนดนโยบายภายนอก 6 คน กลุ่มผู้บริหารภายใน 6 คน กลุ่มผู้ปฏิบัติงานภายใน 17 คน และประชาชนผู้ใช้บริการแพทย์แผนไทยของโรงพยาบาลวังน้ำเย็น 12 คน รวมถึงทบทวนเอกสารวิชาการและเอกสารอื่นที่เกี่ยวข้อง ในการวิเคราะห์ข้อมูลเชิงคุณภาพ ผู้วิจัยดำเนินการเรียบเรียงข้อมูลที่บันทึกสัมภาษณ์ ด้วยวิธีการถอดเทปรายละเอียดและสรุปความในแต่ละประเด็นของคำถาม นำข้อมูลที่ได้นำมาจำแนกและจัดหมวดหมู่ออกให้เป็นระบบ (Typology and Taxonomy) ตามกรอบแนวคิดในการศึกษา จากนั้นนำข้อมูลที่ได้มาวิเคราะห์ส่วนประกอบ (Componential Analysis) และเนื้อหา (Content Analysis) แล้วนำข้อมูลมาเปรียบเทียบวิเคราะห์สามเส้า (Triangle Analysis) จากนั้นจะนำเสนอรายงานแบบพรรณนาเชิงวิเคราะห์ (Analytical Description)

ผลการศึกษาพบว่า 1) ปัจจัยสู่ความสำเร็จการจัดบริการสุขภาพโรงพยาบาลแพทย์แผนไทยต้นแบบ จังหวัดสระแก้ว ได้แก่ ปัจจัยด้านการวางแผน ปัจจัยด้านการจัดองค์การ ปัจจัยด้านการอำนวยการ ปัจจัยด้านการประเมินผลและควบคุมคุณภาพ และปัจจัยด้านพฤติกรรมผู้บริโภคในการตัดสินใจเลือกใช้บริการแพทย์แผนไทย 2) ปัญหาและอุปสรรคการจัดบริการสุขภาพแพทย์แผนไทยโรงพยาบาลวังน้ำเย็น พบว่า บุคลากรฝ่ายรักษาของงานแพทย์แผนไทยไม่เพียงพอ ระบบเทคโนโลยีที่ใช้เก่าและล้าสมัย การประสบปัญหาขาดทุน ข้อจำกัดทางด้านกฎหมาย การใช้จ่ายงบประมาณในการพัฒนาปรับปรุงคุณภาพบริการทำได้ค่อนข้างยากลำบาก ประชาชนยังขาดความรู้เกี่ยวกับการใช้สมุนไพรไทยอย่างถูกต้องปลอดภัย ชาวต่างชาติสนใจประกอบอาชีพนวดไทย การนวดไทยและสมุนไพรไทยเข้าข่ายถูกละเมิดทรัพย์สินทางปัญญา งานวิจัยเชิงประจักษ์ที่สนับสนุนยังมีน้อย

ข้อเสนอแนะ ควรปรับปรุงกฎหมายให้เอื้อต่อการบริหารโดยเฉพาะในด้านการจัดจำหน่ายยาสมุนไพร และควรสร้างประสบการณ์ที่ดีเกี่ยวกับการแพทย์แผนไทยเพื่อให้ผู้ใช้บริการเกิดความเชื่อมั่นและเกิดการบอกต่อไปสู่ผู้อื่น

This was a study into success factors of health service management of model Thai traditional medicine hospital in Sa Kaeo Province. The study aimed at studying into the success factors, problems and obstacles of Thai medicine health service management of Wang Nam Yen Hospital, Sa Kaeo Province. The qualitative research was applied with the use of in-depth interview with 41 key informants classified into 4 groups including 6 external policy makers, 6 internal policy makers, 17 operators, and 12 people who used Thai traditional medicine service of Wang Nam Yen Hospital. Also, the academic documents and other related papers were reviewed. In analyzing qualitative data, the researcher had compiled data obtained from the recorded interviews by transcribing the details and concluding the gist in each question item; then the data obtained were systematically classified and grouped (Typology and taxonomy) under the study framework. After that, the components of data obtained and contents were analyzed (Componential analysis and content analysis), then triangle analysis was performed. Finally, the report was presented on basis of analytical description.

It was found from the study that 1) the success factors of health service management of model Thai traditional medicine hospital in Sa Kaeo Province included factors on planning, organizing, directing, controling, and consumers’ behavior. 2) The problems and obstacles of Thai traditional medicine in managing Thai traditional medicine health service of Wang Nam Yen Hospital were found that there was insufficient number of treating personnel whose work was related to Thai traditional medicine, the technology used was old and out-of-dated, there were problems of loss, legal limitation, difficulty in spending budget to develop service quality, the people lacked knowledge related to how to use Thai herb correctly and safely, foreigners became interested in conducting Thai massage career, and the intellectual property of the Thai massage and Thai herb was violated, there was little supporting empirical research.

The suggestions included: the law should be updated to facilitate the management especially the distribution of herbal medicine, and good experience should be created related to the Thai traditional medicine among service users so that they would feel confident and trust and thus telling others.
